Unipolar Depression Versus Bipolar II Disorder

Unipolar depression is a mental health condition characterized by persistent feelings of sadness, hopelessness, and a lack of interest or pleasure in activities. Symptoms may include changes in appetite, sleep disturbances, fatigue, difficulty concentrating, and thoughts of death or suicide. 

On the other hand, bipolar II disorder is a mental health condition characterized by cycling between depressive and hypomanic episodes. Symptoms of depressive episodes are similar to those present in unipolar depression whereas symptoms of hypomanic episodes include elevated mood, increased energy, decreased need for sleep, grandiosity, and racing thoughts.

Due to the overlap of symptoms between unipolar depression and depressive episodes of bipolar II disorder, the latter is often misdiagnosed as the former. In addition, how hypomanic episodes are less obvious than manic episodes and how individuals may not always realize or disclose their hypomanic symptoms during clinical assessments contributes to the relatively common process of misdiagnosis. In fact, as per the survey taken by the National Depressive and Manic-Depressive Association, 69 percent of patients with bipolar disorder are misdiagnosed initially and more than one-third remained misdiagnosed for 10 years or more.

However, this misdiagnosis can have serious consequences. Particularly, antidepressants as a treatment method for unipolar depression pose a risk of inducing hypomania in patients with bipolar II disorder by excessively elevating mood and energy levels. Research also suggests that antidepressants can speed up mood cycling, which may increase periods of depression and hypomania and exacerbate symptoms. To address these challenges, healthcare professionals must conduct thorough assessments, gather comprehensive medical histories, and involve input from the individual. Increased awareness and education about bipolar II disorder can also contribute to more accurate diagnoses and appropriate treatment plans.
