E-cigarettes, originally invented with the goal of offering a less harmful alternative to traditional cigarettes, were first introduced in the early 2000s by a Chinese pharmacist, Hon Lik. With their fruity flavors and sleek design, these devices have become appealing to teenagers and young adults. An estimated 1.63 million students are currently using e-cigarettes, prompting concern among health officials.
E-cigarettes contain e-liquid — a mixture of nicotine, flavorings, and other chemicals — which is heated into an aerosol and inhaled by the user to deliver nicotine. Some of the compounds formed during this process are either known carcinogens or capable of causing lung damage. Nicotine, the substance responsible for the pleasurable effects of vaping, is a naturally occurring chemical compound found in tobacco leaves. Chemically, nicotine is a small, nonpolar molecule that easily crosses the blood-brain barrier. It acts as a stimulant primarily by binding to nicotinic acetylcholine receptors (nAChRs) in the brain and other parts of the body. This binding triggers the release of dopamine, a neurotransmitter associated with pleasure and reward. The result is a cycle of addiction, as users seek to repeat the pleasurable experience.

Although e-cigarettes are viewed as less harmful than traditional cigarettes, they can have serious health implications. The chemicals in e-cigarettes are known to cause respiratory problems, lung diseases, cardiovascular issues, and cognitive changes in adolescents. Additionally, e-cigarette use is associated with various social impacts, including effects on mental health and disruptions to social interactions.
In South Korea, the sale of nicotine-containing e-cigarettes is prohibited to minors under the age of 19. However, the regulations are not as comprehensive as those for traditional cigarettes. While e-cigarettes are not illegal or classified as drugs, their chemical nature raises ethical and social concerns for young users. This raises the question of how much we truly understand about the chemistry behind e-cigarettes.
