Beyond Medication: Rethinking Treatment Strategies for Antisocial Personality Disorder
오수정 강남포스트 학생기자 | 승인 2025.08.28 20:39
Despite decades of research, pharmacological treatment for antisocial personality disorder (ASPD) remains fundamentally limited. No medications are specifically approved by the FDA for ASPD, and the available evidence on drugs—such as antiepileptics, antidepressants, or dopamine agonists—is consistently weak, largely due to small study samples, short follow-up, and methodological flaws. Most drug therapy is aimed at managing comorbid issues like impulsivity, aggression, or substance misuse rather than core symptoms of ASPD itself. Even where drugs such as SSRIs, mood stabilizers, or antipsychotics are prescribed, findings are inconsistent and often unreplicated; addictive drugs like benzodiazepines are generally avoided because of the high risk of misuse.

Systemic obstacles further complicate care: clinicians often maintain therapeutic pessimism, leading to limited engagement and little investment in developing new treatments. The core behaviors tied to ASPD—notably manipulation and resistance to authority—make forming a therapeutic alliance and ensuring medication compliance uniquely challenging. For these reasons, effective management still depends almost entirely on psychosocial and behavioral programs, which themselves face major hurdles and often have limited success.

To overcome these limitations, expert consensus calls for much larger, longer, and better-controlled research trials focused specifically on ASPD. Outcomes should be measured in real-world terms, like improvement in functioning or reductions in reconviction rates, rather than just symptom checklists. While medication may work best as an adjunct within integrated, holistic care—combined with evidence-based psychosocial therapies such as CBT or DBT—the direct benefits remain modest. Investigating targeted neurobiological mechanisms (especially serotonergic deficits) could offer pathways to more personalized therapy in the future.

Ultimately, progress will require not only rigorous science and multidisciplinary collaboration, but also stigma reduction and clinical training focused on respectful, therapeutic engagement. Improved access to care, ongoing research, and continual innovation remain essential for advancing the management of ASPD.
